How to register - Step By Step -

How To Register for Services

  • Decide on services
  • Read our Policies 
  • Complete the Intake Form (see below) and return it to us by email to request reservations. Specify what services you require. Be mindful of weight limits for hips and elbow x-rays.
  • Make 50% Payment. See Below.
  • Be aware that the OFA  charges a separate fee for evaluation and/or posting.
  • If your dog is getting an eye exam, download and fill out CAER OFA Forms  https://www.ofa.org/application-forms
  • All other forms will be mailed to you. Forms need to be filled out and returned to us to with a 50% down payment to secure the reservation. You may use the drop down list below or the pay pal link. We also offer Venmo and Square for payment. Please send us an email to set up a square taken over the phone. 
  • Send completed forms to health_clinic@silverstateservicedogs.com
  • Make full payment by the due date, February 18th, 2023.
  • Verify paperwork and appointment.
  • See  you there! Getting this all done ahead of time makes the day go much smoother.

Reservations and Payment

 In order to make a reservation, please do not contact the hospitals that these veterinary specialists are affiliated with to schedule OFA appointments. 

Appointments requests can be made via email at: health_clinic@silverstateservicedogs.com. 

You may also call 702-499-1551 and leave a message with the requested information. When requesting appointments please provide: your name, the number of animals, the breeds, and services requested for each.  Also your preferred times. Appointments scheduling is first come first serve. We will make our best attempt at coordinating appointment times if your animals need multiple exams, but there is no guarantee. We will work to make appointments as close together as possible.

Payment and Refunds

Appointments are reserved after the down payment of 50%. 

The remaining payment is due by February 18th, and reservations may be canceled for failure to submit payment.


Policies

 ❑Appointments are reserved only after submitting all paperwork and down payment of 50%

 ❑Deposits are nonrefundable 

❑Failure to submit final payment by February 18th (2 weeks before the clinic date) may result in cancellation

 ❑To qualify for hips and elbows, your dog must be less than 90 pounds 

❑Owner is responsible for separate OFA fees (not included in cost of clinic services) 

❑Doberman owners are responsible for Holter Addendum fee (not included in cost of clinic services) ❑Failure to submit proper paperwork may result in cancellation. 

Check-In

Please arrive 15 minutes prior to your scheduled appointment time to allow for check-in. Dogs receiving eye exams need to be 20 minutes early to apply necessary eye drops before exam. The following breeds are recommended to have a preliminary examination prior to initial pharmacological dilation to best facilitate identification of certain disorders:  Australian Shepherd Dalmatian Louisiana Catahoula Leopard Dog Miniature American Shepherd Miniature/Toy Australian Shepherd

Size Limits

 Yes, your pooch must be less than 90 pounds to qualify for the clinic for hips and elbows. There will be limited staff and we do not have the ability to accommodate large breeds for x-rays. 

Speaking with the Doctors

Due to the volume of animals we will be examining on this day, you WILL NOT speak to the doctor unless your dog DOES NOT clear the exam. Depending on the circumstances, the doctor may request to call you at a later time to discuss their findings. You will be given the results, but it will not be possible to speak at length with the doctor.

Uncooperative Dogs

If your dog does not cooperate during x-rays, we will not be able to complete the service. 

Dogs with Heart Disease

 This clinic is for the clearance of breeding dogs or enthusiasts with performance animals. This is NOT for dogs with known heart disease.

PLEASE DO NOT CONTACT THE HOSPITALS THAT THE VETERINARY SPECIALIST IS AFFILIATED WITH TO SCHEDULE OFA APPOINTMENTS; the staff at the affiliated hospital(s) are not involved with these health screenings. 

All correspondences regarding this and future breeding clinics must be through this email . When requesting appointments please provide: your name, the number of animals, the breeds, and services requested for each.

 Please bring a copy of your dog’s AKC registration form and any identification information you may have for them, incase there are problems with the paperwork. By the day of the clinic, all paperwork should already be submitted.

 In order to book an appointment half of the total service fees are required at the time of booking. Payment in full is required two weeks prior the date of the clinic.
