All of Dr. Boillat's forms are filled out online. These forms are submitted by their office. If you want an evaluation from the OFA or to post your results online by the OFA, you must pay the OFA additional fees. Place your card information at the bottom of all orthopedic tests to send to the OFA.
· Dr. Anne Beyer eye forms are also completed online, then downloaded and provided to us. You will be sent instructions how to complete forms online once you registedr for the clinic. We will print formsand have them for you at the clinic.
·b forms for the echocardiogram are filled out by us and given to you the day of the clinic. You will be given the form back to submit yourself. You send us the information using the intake form. Fill out the form for auscultation (basic cardiac) online. See instructions below.
· Forms need to be filled out and returned to us with a 100% payment to secure the reservation. You may use the PayPal link. We also offer Venmo and Square for payment. Please send us an email to set up a square taken over the phone.
OFA fees are separate. For the clinic, the OFA fees are $15.00 per test. However, on the day of the clinic, you will be given a discount code for all tests except the hips/and elbows. The OFA does not offer a discount for any test that involves an x-ray. The code will cover eyes, heart, thyroid, dentition, and patella. So, for those tests, the OFA fee is 10.00, a five dollar discount per test.
You may apply the discount to every eligible test performed. So, if you have 5 thyroids and want them sent in and posted on by the OFA, the discount is $25.00.
For the hips and elbows, the OFA fee is $30.00 or $15.00 for either test and there is no discount given by the OFA.

A veterinary cardiologist. Veterinary cardiologists are defined as licensed veterinarians with diplomate status in the American College of Veterinary Internal Medicine (ACVIM) cardiology sub-specialty.

Please arrive 15 minutes prior to your scheduled appointment time to allow for check-in. Dogs receiving eye exams need to be 20 minutes early to apply necessary eye drops before exam. The following breeds are recommended to have a preliminary examination prior to initial pharmacological dilation to best facilitate identification of certain disorders: Australian Shepherd, Dalmatian, Louisiana Catahoula Leopard Dog, Miniature American Shepherd, Miniature/Toy Australian Shepherd
Yes, your pooch must be less than 90 pounds to qualify for the clinic for hips and elbows. There will be limited staff and we do not have the ability to accommodate large breeds for x-rays.
Due to the volume of animals we will be examining on this day, you WILL NOT speak to the doctor unless your dog DOES NOT clear the exam. Depending on the circumstances, the doctor may request to call you at a later time to discuss their findings. You will be given the results, but it will not be possible to speak at length with the doctor.
If your dog does not cooperate during x-rays, we will not be able to complete the service.
This clinic is for the clearance of breeding dogs or enthusiasts with performance animals. This is NOT for dogs with known heart disease.
